Primary Care Networks (PCN's)
Practices have begun working together and with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in Primary Care Networks.
Primary Care Networks build on the core of current primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care. Clinicians describe this as a change from reactively providing appointments to proactively care for the people and communities they serve.
The PCN that Norfolk Street Surgery is part of is called Shelton/Hanley PCN and is made up of the following Practice's:

Five Town's Surgery
Birches Head Medical Centre
Hanley Health Centre
We will be working together to provide community services for our practice populations to improve access to care and services.
Extended Hours Directed Enhanced Service
We will no longer be providing extended hours at the Practice, however as part of our PCN, we will be providing Extended Hours on Saturday mornings along with the other Practice's in our PCN at:
Hanley Health Centre
The Extended Hours will run between 9:00 and 12:45pm and can be booked in advance via the Practice during the week. Appointments will be available with GP's, Nurse's and Healthcare Assistants. So, if you are unable to get an appointment at Norfolk Street Surgery during the week or you are unable to attend during the week due to work commitments etc, we can now offer you appointments at this clinic. Please ask at Reception for more details.
Extended Access Primary Care Programme commenced on 1st September 2018 whereby evening and weekend appointments will be available across Stoke-on-Trent and Staffordshire. Appointments for the Extended Access Service must be booked through the Practice, so please call our Reception to book an appointment. Clinic's are available every evening and at weekends at the following locations:

We are introducing an initiative to help people to find the most appropriate source of help when they call our Practice or pop in to make an appointment.
This means that our specially trained Reception staff may ask questions about why appointments are required.
You are not obliged to answer the questions they ask, however by doing so we can make sure that those people who really need to see a GP can see them as soon as possible.
Virtually everyone who calls to make an appointment asks to see a GP, but very often help will be available more quickly from other highly skilled medical professionals such as Nurses or Pharmacists, or from other services, including the voluntary sector, they can support people with a wide range of social, emotional or practical needs.
Our trained Reception staff will be able to offer you the most appropriate help using the range of information they have access to.

'Ask NHS' app – get involved
Have you used the new Ask NHS App? We want to hear from you! ‘Ask NHS' provides you with improved access to NHS 111 and your local NHS services. You can talk through your symptoms in complete confidence with Nurse Olivia, a virtual nurse. If needed, Olivia will arrange for a call back from a 111 Nurse to discuss your symptoms further. You can also search NHS approved healthcare advice, schedule GP appointments and search opening times/locations of local healthcare services. We would like to know what you think about the app and gather any feedback or comments you may have.
